A mysterious infection is spreading through the city, turning people into demonic versions of how they see themselves. Hyun-Soo himself becomes infected but stays in control. The thing about the demons is they don't die.
Only South Korea was shown in Sweet Home and Shotgun Boy; however, the entire planet was affected. The monster apocalypse was caused when people gave into their desires, causing them to turn into monsters. It is believed to have been started in Shotgun Boy by a monster named Zero.
Hyun-Su got infected due to his desires. So does the woman who lost her baby. As the drama progresses, many characters begin to turn into monsters after they begin to surrender to their desires, perhaps none more vilifying than watching the abusive supermarket man begin to turn.
The hallucination is actually the entity responsible for turning people into monsters by manipulating them to reveal and give in to their greatest desire. The entity is very persistent and will try any means to get the human to give in.
While this did allow her inner monster to take control of the situation, Myeong-Ja was ultimately able to undermine the inner monster's malicious intent by having her desire turn her into a docile infant.
Who is the strongest monster in Sweet Home?
The steroid monster's former human self was unknown, but it can be assumed he was a bodybuilder with a desire to get physically strong. Befitting to its immense and muscular appearance, it possesses extreme strength, far beyond that of any other monster.
It is believed to manifest as a disease where one could possibly get infected through physical contact. But it was later revealed that the phenomenon is rather a curse that humans are destined to experience. Everyone is susceptible from such curse.

Sweet Home K-Drama Review & Summary: A Violent & Engaging Horror. Sweet Home is a K-Drama based on the webtoon series of the same name by authors Kim Carnby and Hwang Young-chan. It is everything you'd expect from a K-Drama thrilling, suspenseful with a pinch of comedy.
Obviously this being a horror drama, the romance didn't go anywhere but the subtleness of it left an impact. They did transfer over the same love line between Sang Wook and Yu Ri and that one felt very natural.
Based on the Korean Webtoon by Kim Kan-bi and Hwang Young-chan, Sweet Home is based around the life of Cha Hyun-soo. Having recently lost his family in a tragic accident, Cha moves into an apartment in an attempt to begin a new life.
